Job Overview

St.Ives Specialist Hospital is seeking a dedicated and experienced Pharmacist to join our esteemed institution in a remote capacity. This critical role will involve the remote provision of comprehensive pharmaceutical services, ensuring the safe, effective, and appropriate use of medications for our diverse patient population. You will be an integral part of our healthcare team, contributing to optimal patient outcomes through expert medication management and patient education, all while working from the convenience of your own home.

Key Responsibilities

* Review and dispense prescriptions accurately, ensuring compliance with all legal and hospital policies and procedures.
* Provide expert clinical consultation to physicians, nurses, and other healthcare professionals regarding drug selection, dosing, interactions, and contraindications.
* Monitor patient medication regimens for effectiveness, safety, and adherence, intervening as necessary to optimize therapy.
* Educate patients and their families on the proper use of medications, potential side effects, and storage instructions in a clear and understandable manner.
* Maintain comprehensive and accurate patient medication records, documenting all interventions and consultations.
* Participate in the development and implementation of hospital-wide medication use policies and protocols.
* Identify and report medication errors and adverse drug events, contributing to a culture of patient safety and continuous improvement.
* Stay abreast of the latest advancements in pharmaceutical practice, therapeutic guidelines, and relevant medical literature to ensure the highest standard of care.

Requirements

* Doctor of Pharmacy (Pharm.D.) degree from an accredited institution.
* Current, unrestricted license to practice as a Pharmacist in [Specify State/Country if applicable].
