Job Overview

We are seeking a highly skilled and dedicated Community Health Extension Worker (CHEW) to join our team at Krystal Medical Centre. As a CHEW, you will play a vital role in promoting health education, disease prevention, and community outreach programs that aim to improve the overall health and well-being of our clients. Your expertise and experience will be instrumental in developing and implementing community-based health initiatives, fostering partnerships with local healthcare providers, and delivering high-quality patient care. You will work closely with our multidisciplinary team to ensure seamless continuity of care and provide support to clients in remote settings.

Key Responsibilities

* Develop and implement community-based health education programs that address the unique needs of our clients
* Conduct health assessments, identify health risks, and provide personalized guidance on disease prevention and management
* Collaborate with local healthcare providers to promote health education, disease prevention, and community outreach programs
* Deliver high-quality patient care, including health screenings, vaccinations, and basic medical treatments
* Foster partnerships with local community groups, organizations, and healthcare providers to promote health education and disease prevention
* Develop and maintain accurate records of client interactions, health assessments, and community outreach programs
* Participate in quality improvement initiatives to enhance community health outcomes and patient satisfaction
* Provide emotional support, counseling, and guidance to clients and their families, as needed

Requirements

* Diploma or degree in Community Health, Nursing, or a related field
* Proven experience working as a Community Health Extension Worker or in a related role
* Strong knowledge of community health principles, disease prevention, and health promotion
* Excellent communication, interpersonal, and counseling skills
* Ability to work independently and as part of a multidisciplinary team
* Strong problem-solving and critical thinking skills, with the ability to make sound judgments in high-pressure situations
* Familiarity with electronic health records and other healthcare software applications
